Photos - Submission by mail or hand delivered to MHS

Photo Submission Form

 For each 1/8th of a page, you may submit up to two (2) photographs.  Follow these guidelines for photo submissions:

  • Submit electronic files of your pictures on a CD.
  • Files must be JPG format
  • Files must be 300 DPI or larger
  • IMPORTANT! File names must be in the following format: Lastname_firstname1.jpg, Lastname_firstname2.jpg, etc.

A photo may be submitted by formprinting this form and submit photos on a compact disk.

PLEASE DO NOT SUBMIT HARD COPIES OF PHOTOS; THESE CAN NOT BE RETURNED.
But if you would like to come to L205B you may use our scanner.

Text - TYPE IT OUT

You may submit no more than 175 letters and spaces of text for each 1/8th of a page.  Please, do not exceed 175 letters and spaces total for each 1/8th page.  Attach a type written copy of the text to the form.  Please note, the Yearbook staff reserves the right to edit ads for grammatical conventions and length, if necessary.

Yearbook staff members will format the ad. Please do not submit formatted ads!

Payment- WRITE A CHECK

If you’re not submitting the ad on line, please submit payment in a check, made out to Monarch High School.  Please do not submit cash! Payment must be included with order.

Place this completed form, your check, and your CD (with your senior’s first and last name written on it with a permanent marker) in an envelope and submit to Mrs. Katzive, room L205B by October 2, 2015.

Ads may also be submitted to the Treasurer’s Office (outside the main office).

Ads submitted after this date will be charged a $10.00 late fee, one to two weeks past the deadline.
